D.O.M.S 😢 ➖ Delayed onset of muscle soreness (DOMS) is the pain and stiffness felt in your muscles after unaccustomed or strenuous exercise🤓 • DOMS is felt most strongly 24 to 72 hours after the exercise. The mechanism is not completely understood, but the pain is ultimately thought to be a result of micro-trauma (mechanical damage at a very small scale) to the muscles being exercised. • After such exercise, your muscle adapts rapidly to prevent soreness if and when the exercise is repeated. This is called the repeated-bout effect”, and is the reason you don’t get the same intensity of soreness each time. Which is great news for us all 👏🏼! • DOMS can be reduced by ensuring that you gradually increase the intensity of a new exercise program. [ HIP BURSITIS ] --- Lets talk bursitis. A common DM I get is How do I fix X condition. Hip bursitis seems to get tossed around a lot. A better classification for it is Greater Trochanteric Pain Syndrome (GTPS) If youre a patient or someone who has GTPS, the naming probably is the least of your worries. But I think this quote from a systematic review of the literature should be quite positive for most: There is currently no evidence-based protocol for the management of GTPS. Conservative treatment is the gold standard for GTPS with over 90% success rate. (Reid 2016) What does that translate into for you: the prognosis is quite favorable for resolution in the long term. I tend to favor a progressive loading protocol for most things, and GTPS is no different. My process is quite similar to any tendinopathy: initiate loading, examine symptoms on a 24-48 hour timeline, and modify based on response. That said, thats an overly simplified condensation of how I approach it. Success is rarely a straight line. 1️⃣ Improve hip ROM 2️⃣ Strengthen muscles surrounding the hip 3️⃣ Closed chain loading - 🔑  Take home point: Most cases of GTPS should resolve. This exercise does a particularly good job at loading the lateral hip musculature which plays an important role in stabilizing the hips in single-leg stance, and so on. . 💪Carrying such heavy weight ensures not only that an athletes lower body is working, but requires adequate grip strength, shoulder stability, and core involvement so safely transport the weight. . 🚶‍♀️Walking SLOWLY is the key here, with small and precise steps. Maintain a level and stable pelvis... and remember to BREATHE! . 🏃‍♀️Athletes should try to find a weight that is challenging, around a 5-7/10 in terms of difficulty, and try to perform 3x30 walks working up to 1 minute walks. . We picked several positions of his throwing motion and had his shoulder in two positions- short lever (arm bent) and long lever (arm extended) for each position. The goal here is to reinforce tissues at their end ranges of motion so that we can increase capacity at that range, thus mitigating injury in the future. I also wanted him to use his entire body interdependently to generate way more tension and self organize as he engaged.
